What You’ll Do
As a PRN Home Health PTA, you’ll partner with our Physical Therapists to deliver personalized, high-quality care—directly in patients’ homes.
- Provide one-on-one physical therapy treatments in home health settings
- Implement individualized plans of care under the supervision of a Physical Therapist
- Perform therapeutic exercises, manual therapy, gait training, balance activities, and functional mobility training
- Monitor patient progress and communicate updates clearly to the supervising therapist
- Educate patients and caregivers on home exercise programs, safety, and fall prevention
- Complete timely, accurate documentation in compliance with home health regulations
- Assist with discharge planning to support smooth transitions of care
What We’re Looking For
- Active Physical Therapist Assistant (PTA) license in good standing
- Home health experience preferred (new grads with strong clinical skills welcome)
- Ability to work independently and manage visits efficiently
- Strong communication and patient education skills
- Reliable transportation and willingness to travel within Santa Clarita, Northridge, and surrounding areas
