Overview
We are seeking a dedicated and detail-oriented Skilled Nursing Facility Rounds professional to provide comprehensive patient assessments and coordinate care within a skilled nursing environment. The ideal candidate will possess strong clinical skills, a thorough understanding of physiology and anatomy, and a commitment to delivering high-quality patient care. This role involves conducting regular rounds, monitoring patient health status, and ensuring compliance with healthcare regulations, including HIPAA privacy standards. The successful applicant will play a vital role in maintaining accurate medical documentation and supporting interdisciplinary team efforts to promote optimal patient outcomes.
Duties
- Conduct routine rounds in skilled nursing facilities to assess patient conditions, monitor vital signs, and identify any changes requiring intervention.
- Collaborate with nursing staff, physicians, and other healthcare professionals to develop and update individualized care plans.
- Document all patient interactions, observations, and care activities accurately within electronic medical records while adhering to HIPAA regulations.
- Review medical records for completeness, accuracy, and compliance with regulatory standards; ensure proper handling of sensitive information.
- Apply knowledge of physiology and anatomy to evaluate patient health status effectively and recognize early signs of deterioration.
- Educate patients and their families about treatment plans, medication management, and health maintenance strategies.
- Ensure that all medical documentation is thorough, timely, and compliant with legal and institutional policies.
- Maintain a professional environment that emphasizes patient safety, confidentiality, and compassionate care.
Qualifications
- Proven experience in nursing or healthcare settings with a focus on patient care within skilled nursing facilities.
- Strong knowledge of physiology, anatomy, medical terminology, and clinical assessment techniques.
- Familiarity with medical documentation practices and electronic health record systems.
- Understanding of HIPAA regulations related to patient privacy and data security.
- Excellent communication skills for effective interaction with patients, families, and interdisciplinary teams.
- Ability to perform detailed assessments and recognize early warning signs of health issues.
- Certification as a licensed nurse or relevant healthcare credential is preferred; relevant experience may be considered in lieu of formal licensure.
