Physician Assistant – Physiatry (Skilled Nursing Facilities) Job Type: Full-time, W2 Work Location: In-person at the facility (multi-site rotation) Medical Specialty: Physical Medicine & Rehabilitation (PM&R), Geriatrics, Long-Term Care, Post-Acute Medicine Supplemental Pay: Monthly bonus opportunities

Are you a Physician Assistant with a passion for rehab medicine, functional outcomes, and geriatric care?

As a Physiatry Physician Assistant, you will work in direct collaboration with a Physiatrist (MD) while supporting rehabilitative and medical management of patients recovering from orthopedic procedures, neurologic events, and other function-limiting conditions. You will play a critical role in optimizing recovery, reducing re-hospitalizations, and supporting patients’ return to independence.

Compensation & Benefits

  • Annual Salary: $110,000 – $130,000 based on experience

Technology

Job Requirements

  • Active and unrestricted Physician Assistant license in Illinois

  • NCCPA certification required

  • Experience in PM&R, orthopedics, neurology, geriatrics, internal medicine, or SNF/post-acute preferred

  • Strong clinical skills in functional assessments, medical management, and rehab-aligned care planning

  • Comfortable practicing with physician collaboration yet strong autonomy in routine patient management

  • EMR proficiency and commitment to timely documentation and coordination

Key Responsibilities

In this role, you will serve as a functional medicine specialist within the Skilled Nursing Facility:

  • Conduct comprehensive medical and functional assessments

  • Develop, adjust, and co-manage rehab treatment plans in partnership with the supervising Physiatrist

  • Lead medical follow-ups to monitor progress, optimize pain management, and support safe mobility progression

  • Participate in interdisciplinary care conferences and team rounds

  • Collaborate with physical, occupational, and speech therapy teams to track and support rehab milestones

  • Educate patients and families on diagnoses, recovery timelines, and functional expectations

  • Complete thorough and compliant EMR documentation in a timely manner

  • Support safe discharge planning and care transitions

  • Serve as a trusted consult resource for musculoskeletal and neurologic concerns within the facility
