Immediate Locum Opportunity: Nurse Practitioner / Physician Assistant (PACE Program)
Duration: Approximately 2–4 monthsLocation: Greater Los Angeles
About the Role

As a Locum Nurse Practitioner or Physician Assistant, you will be an integral member of our Interdisciplinary Team (IDT), providing direct medical care to PACE participants across a variety of settings — center, clinic, home, virtually, or external sites of care.
You will collaborate closely with the IDT to develop and implement individualized care plans that improve participant health outcomes and support whole-person, dignified care.
Key Responsibilities
- Serve as a core member of the IDT; complete assigned portions of participant assessments, care plans, and service determination requests
- Actively manage and support a panel of participants
- Initiate, evaluate, modify, and review orders for medications, diagnostics, and treatments (as approved)
- Assess, initiate, and coordinate participant care daily under appropriate supervision
- Document all participant interactions, assessments, and care plan steps in the EMR within required timeframes
- Make specialist referrals (as approved) and request consults as needed
- Perform history and physicals, participant education, and selected procedures
- Order, interpret, and follow up on labs, diagnostics, and results
- Identify, escalate, and follow up on clinical concerns
- Collaborate closely with behavioral health, pharmacy, and other IDT members
- Provide care in multiple settings: center, home, virtual visits, skilled nursing facilities, and more
- Facilitate advance care planning conversations with participants and families
- Accurately document chronic conditions (HCC capture)
- Participate in on-call coverage as needed
What We’re Looking For
- Passion for serving high-risk seniors and frail older adults
- Experience delivering care to complex and older adult populations
- Strong communication skills and the ability to lead independently while collaborating effectively within a team
- If NP: Minimum of 2 years’ RN experience with complex populations prior to NP transition
- Required Credentials:
- Active, unrestricted California NP or PA license
- If NP, active RN license in California
- Master’s degree and graduation from an accredited NP program (if NP)
- Certificate as a Nurse Practitioner with prescriptive authority (if NP)
- CPR certification
- Valid CA driver’s license, reliable transportation, clean driving record, and auto insurance
- Knowledge of chronic care and geriatric best practices
- Bilingual English/Spanish strongly preferred
Pay: $110.00 – $130.00 per hour
Expected hours: 40 per week
