Key Responsibilities:
- Perform comprehensive health assessments for adult patients.
- Manage acute and chronic conditions, including common and complex cases.
- Order and interpret diagnostic tests to guide care decisions.
- Develop individualized care plans and prescribe appropriate treatments.
- Collaborate with a supportive, multidisciplinary team to optimize patient outcomes.
Requirements:
- Experience: 2+ years in Adult Primary Care preferred.
- Licensure: Advanced Practice License in Connecticut (or eligibility).
- Certification: Current Nurse Practitioner certification.
- DEA License: Active or eligibility for registration.
